If you are not in critical condition, and if time permits, it is best to plan and prepare for your hospital stay with the following simple reminders:
- Prepare your identification cards indicating personal information, such as your current address, phone number, next of kin, and employer. It is best to bring a valid government-issued photo ID.
- Organize your health information—your condition, medical records, as well as your current medications (prescription, over-the-counter, and herbal). Keep this, along with personal information, in an updated list in your wallet so it will be readily available in case of emergencies.
- Plan for your payment arrangements. Bring your insurance documents or medical coverage ID cards, as well as the name and contact number(s) of your doctor(s), whether or not they are practicing at MakatiMed.
- Bring your healthcare cards or insurance coverage forms, as well as any special form(s) or pre-authorization documents such as Letter of Guarantee (LOG) or Letter of Authority (LOA).
- If you are married, bring information about your spouse’s insurance coverage to ensure that you avail yourself of full insurance benefits as his/her dependent.
Deposit
A reasonable amount is required as initial deposit during a hospital admission at Makati Medical Center. For non-urgent confinement, the deposit is required to be settled upon admission, or, in case of urgent or emergency confinement, as soon as the patient is stable. This will be discussed by MakatiMed’s Admissions and/or Credit & Collection Officers.
Deposits are not required for patients bearing a Letter of Authorization (LOA) or Letter of Guarantee (LOG) issued by the patient’s insurance company, health maintenance organization (HMO), or corporate affiliation as its member, dependent, or beneficiary. If the patient does not have the LOA or LOG at the time of admission, the patient is required to pay the deposit, which will be refunded upon discharge.
As the hospital bill amount increases, the patient will be requested to make additional deposit(s) during the hospital stay.
If the patient decides not to proceed with the inpatient admission, Makati Medical Center will assist the patient in his or her transfer to another hospital of the patient’s choice.
If the patient does not have a preference, and in compliance with government regulations, MakatiMed will transfer the patient to the nearest government hospital.
Our goal is to help you get settled and feel comfortable in the hospital environment as quickly as possible. We encourage you to have with you the following documents (as applicable):
1. Personal Identification (ID) with photo
- Accredited HMO-issued member’s ID
- Senior Citizen’s ID
- PhilHealth ID
- Persons with Disability ID
- Healthcare insurance card or document
- Digitized SSS ID
- GSIS ID
- PRC ID
- Integrated Bar of the Philippines ID
- Unified Multi-Purpose ID
- Driver’s License
- Passport
2. Signed Letter of Guarantee (LOG) or Letter of Authority (LOA) from your company (if you are an eligible employee for company healthcare hospitalization benefits)
3. Any prior medical records, X-Ray films or test results (especially if you had a recent hospital confinement at another medical facility or hospital)
4. A copy of your Advance Directives regarding your medical care (if available)
The hospital admissions personnel will issue you a unique patient identification number in a bracelet indicating your full name, date of birth, a patient ID number, and the name of your Attending Medical Doctor (AMD). The identification (ID) bracelet must be worn at all times during your hospital confinement. DO NOT REMOVE the ID bracelet until you are discharged from MakatiMed. This is for your own safety.

The security and safety of your personal property and valuables are important to us. It is vital that you feel at ease and secure during your stay in the hospital. We recommend that you read and familiarize yourself with these Patient Safety/Security Tips:
- Please do not bring expensive electronic equipment, gadgets, jewelries, large sums of cash, or other similar valuable items. MakatiMed will NOT be responsible for the loss of such items.
- The nurse on duty will lock your room whenever you are required to go elsewhere in the hospital for a medical test, examination or procedure. If for any reason you need to leave the room and a nurse is unavailable, kindly lock your room. Our nurses may unlock your room for you upon your return.
- The hospital does NOT authorize any individual to request payment of hospital bills or physician professional fees in a patient’s room. All bills including doctors’ professional fees must be settled at the cashier only. Your HMO Liaison Officer or a representative of your insurance provider may visit or contact you. To ensure that you only speak to authorized personnel, you may contact your nurse to validate the identity of the personnel in case you have not met them during your admission.
- The hospital likewise does NOT authorize any individual to solicit, promote, or advertise any commercial products or items inside the patient rooms.
