  7. Certification of good standing from specialty/subspecialty society and Philippine Medical Association (PMA) – Original Copy
  8. Two (2) pieces 2×2 ID pictures (colored), white background
  9. Two (2) recommendation letters from senior physicians (preferably from Makati Medical Center)
  10. Current PhilHealth ID (Accredited Professional Health Care Provider and Individual Paying Member)
  11. Curriculum Vitae
  12. Valid Basic Life Support (BLS) Certificate
  13. ACLS (Advanced Cardiovascular Life Support) Certificate for Anesthesiology, Medicine, Emergency Medicine, Physical Medicine and Rehabilitation, Neurosurgery, Neurology and Surgery.
  14. PALS (Pediatric Advanced Life Support) /NALS (Neonatal Advanced Life Support) Certificate for Pediatrics
  15. Census of procedures for the last two years with outcome data (those who have surgical and interventional procedures)
  16. BIR Form 2303
