Applications for consultants are handled by Medical Services. Please refer to the list below for the requirements:

  1. Accomplished application form. Application forms are available at the Medical Services Office
  2. Current Professional Regulatory Commission (PRC) License
  3. Current Professional Tax Receipt (PTR)
  4. Current Dangerous Drug Board (DDB) identification card
  5. Photocopy of Medical/Dental School Diploma, Internship, Residency and Fellowship
  6. Photocopy of Certification of Specialty Board
  7. Photocopy of Certification of Subspecialty Board (if applicable)
  8. Certification (original copy) of good standing from specialty/subspecialty society and the Philippine Medical Association (PMA)
  9. Two (2) pieces 2×2 color ID pictures with white background
  10. Two (2) recommendation letters from senior physicians (preferably from MakatiMed)
  11. Current PhilHealth ID (Accredited Professional Health Care Provider and Individual Paying Member)
  12. Curriculum Vitae
  13. Valid Basic Life Support (BLS) Certificate
  14. ACLS (Advanced Cardiovascular Life Support) Certificate for CT-MRI, Neurology, Neurosurgery, Anesthesiology, Nuclear Medicine, Interventional Radiology, Pulmonary Medicine, Gastroenterology, Nephrology, Rheumatology and Cardiology
  15. PALS (Pediatric Advanced Life Support) /NALS (Neonatal Advanced Life Support) Certificate for Pediatrics
