Advance Directives

What kind of medical care would you want if you were too sick or injured to relate your wishes? Advance directives are legal documents that let you state your decisions about end-of-life care ahead of time.

Living Will
A document that describes the type of medical treatment you want in certain situations. It comes into effect if you are terminally ill or permanently unconscious or unable to express / communicate your wishes.

DNR (Do Not Resuscitate) Order
This allows you to ask NOT to have cardiopulmonary resuscitation (CPR) or other treatment to try to revive you if your heart stops or if you stop breathing.

Declaration to Withdraw or Withhold Treatment
Can be made by a patient or relative of a patient to withhold or withdraw life-sustaining procedures that will prolong the dying process.

Durable Power of Attorney
This document states who you choose to speak on your behalf about health care decisions if you cannot do so yourself.
